The radiologist should include gadolinium (contrast) in a new MRI with surgery less than one year old. The prescription generally is not written by the radiologist so make sure whomever writes the script includes gadolinium.

Increased lower back pain could be a larger annular tear, a newer disc herniation or just an aggravation of some of the old tearing of the annulus. Since the pain has significantly increased, watch for signs of increased buttocks pain or paresthesias (pins and needles) in the leg. This could be an indication of a recurrent disc herniation. A new physical examination could shed light on this flair-up. Increased numbness, a new reflex deficit or muscle weakness could indicate a new recurrent disc herniation. If those signs are not present, sometimes as we discussed before, an oral steroid can be used to calm the increased pain.

No, especially with no leg symptoms, you can give time a chance to calm down the flair-up. See if you can get some physical therapy prescribed to help with the symptoms.
