    Hello dr…
I had a micro discectomy on january 14th 2016…its was great i was healing day by day…
After 20 days on feb 5th 2016…i got a severe pain in my leg again…
When i consulted my surgeon he said this happens and it should settle down in 5 days…
It has past 8 days now…my pain is not gone but better then the first day..
When i asked doctor the reason they said the material from the disc can come out..which eventually gets dissolve itself…i asked them about mri…they said post operation 1 month you will not get a clear picture in mri as there is water remaining…
Can u please help me to understand what is happening..and will mri help after 1month of surgery…

    Thnx

Pain relief and then recurrence after microdisc surgery can occur from three problems; recurrent disc herniation, seroma formation or radiculitis (inflammation of the nerve root). I generally will give a patient an oral steroid if this occurs with the expectation that nerve inflammation and seroma could resolve with time and meds. If that is ineffective and the patient is static (not improving), I would consider a new MRI.

Yes, on a post-operative MRI you will see fluid as a small amount of fluid is expected after surgery. You however can see a recurrent herniation and a root that is displaced by too much fluid (compressive seroma). If there is a compressive seroma, you can have your local interventionist place a small needle, drain the seroma and even place some steroid to reduce inflammation.

The steroid will not reduce the size of the seroma but is quite effective in reducing inflammation. Let us know how you do after the course of steroids.
