    Hello,

    Dr Corenman, a short recap of my situation, if I may:
    I am now 3,5 months away since microdiscectomy L5-S1
    1 month post op. MRI shown :
    -bone edema occurrence in the adjacent vertebral plateaus with caudal migrated left paramedian disc herniation L5 compressing the left root S1
    In the meantime the pain on left side has been reduced, but one month ago I started to have pain in my right leg,which I never had before, nerve pain, pain down the leg and in my bottock, like I was having a herniated disk on the right side .
    3.5 month post op. MRI shown:
    -rectilinear lumbar spine, without axis rotations, L5-S1 disc dehydration with preservation of intervertebral space
    -herniated disc fragment postero-median L5-S1
    -Bone edema lower vertebral plateau L5
    -without signal changes at the structural level of the lumbar spine and the posterior articular mass
    -without signal changes in the medullary cone and tail
    -without tumoral formations
    Could you please tell me what do you think, the fragment from left para-median reherniation as shown in the one month post op MRI has shifted to postero-median position? Is the pain that I feel in my right leg because of this ? Is that possible?

    Thanks for the update.
    How bad is your pain on a scale 1-10? Mine is a 4.Do you have pain when walking or when sitting or after sitting? When you say your leg is badly in atrophy , what exactely does that mean, do you feel it tight when walking?
    I’m asking you because a lot of days during my recovery/after reherniation, especially when walking I felt this way, my leg was feeling like it was shorter, and something was keeping it tight in my bottock(I don’t know how to explain this better).Now it’s better but I still feel it that way sometimes.
    I’m also considering cortison injections, please let me know if you feel better after the first one.
    After you check with the therapist could you please share with me what kind of exercises he ore she recommended you?

    You didn’t mention how many weeks have past since you had the microdiscectomy.
    As you said you might still have symptoms from the first herniation and the surgery.
    Maybe is too soon for PT as your surgeon told you.
    Iniitially my doctor recommended to start PT two weeks after the surgery, but then telling him that I still have bottock pain and leg pain he told me to wait, then when it was one month after the surgery and I made the MRI he saw the new herniation and he told me that I can start PT if I don’t have intense pain, because I’m not at risk…
    Same as you, I have doubts about how PT could help in this kind of situations, I always had then even before the surgery especially because for me it didn’t work before the surgery…
    I never had an epidural, have you? As dr Corenman said, it might help.

    Hello Sebastian,

    Sorry to hear that you are in a similar situation.
    I was waiting for doctor Corenman to reply, if he had any similar cases, if it’s really possible for the fragment do dehydrate and what exactely that means, besides the fact that it might shrink, because in my logic, if it will dehydrate that equals to calcification, but is just my assumption.
    Let’s say it will dehydrate , I was wandering if it’s safe to live with it there , what that means for the future?Maybe your surgeon answered to this kind of questions?
    I don’t know how big is your reherniation, mine is bigger than the first one,but like Dr Corenman said in previous replies , it’s important where is the herniation located in the canal, the symptoms are related to the location.
    Since I’ve wrote the last reply, my symptoms improved a little bit, I don’t know if it’s because I started to exercise or because it’s dehydrating…
    I’m planning to make another MRI in a few months to see exactely what’s going on there, I’m really affraid of another operation considering what happened.
    I hope your condition will improve ,I know it’s hard but we need to be patient for now I guess .

    Hello Dr.Corenman,

    The pain is tolerable and it doesn’t prevent me from walking, however 4 days ago I’ve started to exercise(kinetotherapy) and the pain intensified a little bit , especially when I’m sitting.Is this normal?
    If I may, I want to ask you another thing , is it possible for the herniated fragment to dehydrate, so it may shrink over time, as my surgen told me?
    Have you encountered this in your career?
