My preoperative diagnosis – Lumbar radiculopathy, lower extremity weakness, lumbar
stenosis, isthmic spondylolisthesis, low back pain.
Procedure performed on March 21, 2017
L5-S1 decompression, Gill fragment removal of L5,
bilateral facetectomy, removal of L5-S1 disk through a transforaminal approach
with placement of bone graft and BMP in the interbody space, bilateral
placement of L5 and S1 pedicle screws with rod and harvesting of local
autograft with placement into the lateral gutters along with bone morphogenetic
proteins as well as interpretation of intraoperative films.
