Different states have different regulations. Unfortunately, with workman’s compensation cases, there are a very few patients who try to “game” the system and this is why the rules are so arbitrary. The system also varies state by state. Colorado is actually reasonable for work comp treatment. I know California has some crazy rules. I do not know how New York rules work.

If the injectionist used large volumes of lidocaine, there is the possibility that some of this volume “leaked down” to another level and the test is a false positive (pain relief of the pars when really the pain relief is of another structure at a different level).
It is possible but less likely that this injection will yield a “cure”. Loading the pannus again in the normal course of your duties as a firefighter will probably aggravate the pain nerves. Nonetheless, if you really want to return to firefighting, you should consider starting to load your spine in the controlled environment of physical therapy. An aggravation again will indicate you should either consider surgery if you and your surgeon are convinced that the pars fractures are truly your pain generators or find another line of work that is less taxing to your spine.

End plate changes lead to a diagnosis of isolated disc resorption, a condition where the disc no longer can absorb impact and the stress is subsequently transferred to the endplate. The endplate then develops painful small fractures.
Ozone therapy is not a treatment I recommend. Ozone is oxygen in the O3 state (not the normal O2 state). In the O3 state, oxygen is highly reactive and will oxidize any tissue it comes in contact with. Also, injecting gas, even in solution into tissues can lead to embolism.

This means that there is a narrowing of the foramen or the lateral recess (see website) and with the added normal narrowing that occurs with standing and walking, this activity causes nerve compression and leg pain.
Back pain can be generated by degenerative discs, degenerative facets, instability (the inability of the back to resist normal motions) and stenosis.
If you are still contemplating surgery, you would need a work-up to determine your pain generators. This work-up would possibly include nerve blocks, facets blocks and possibly discograms (see website for descriptions).

I generally do not recommend specific surgeons on the website. In the UK, there are some very good spine surgeons. Google the ones you are interested in to gain more information.
