I am going in for revision surgery for acdf on c5-c7. I was told because I didn’t quit smoking the bone didn’t grow which I stopped now. I accept that is why. My ? Is that I have lupus, spondylosis, DDD, scoleosis, neropathy, myositis I think it is, and the screws from my last surgery are backing out, is it possible that my other diseases had a part in my fusion not succeeding? I also have adrenal insuff. And heart troubles as well. Thank you have a wonderful evening

If you have had a pseudoarthrosis (lack of fusion) of C5-7 after an ACDF (fusion), smoking is probably one of the pain reasons for this. If you are going to have a revision, I would advise using your own hip graft as the replacement graft. This gives you the highest chance of fusion. With Lupus (SLE), make sure that you are taking enough protein. This can be checked with some lab tests. Take Vit. D and calcium/magnesium supplements. If you have osteoporosis or real problems with bone healing (checked by a dexa scan) you can take Forteo, a parathyroid hormone analog (off label use) which can speed healing.

Thank you very much Dr. I appreciate you taking time to answer. I have anal insufficiency and was on prolonged steroids over a year that affected by bones I haven’t had other tests to check for any other bone diseases. I do know I have degenerative disc disease I know in my neck and back. They’re more concerned with my due to being the main control center of nerves I guess. But my lumbar pain is seriously affecting my activities

Long term steroids will decrease the chance of fusion and need to be countermanded. I would definitively use your own bone for this fusion and get a consultation for Forteo.
