Thank you for your response in the past.

I am 2 years postop of a acdf C5-7… unfortunately, i dont have the report on hand for xrays to see if theirs a non union. My recent report of my Ct scan states:

CT of spine demonstrates post surgical changes with anterior fusion hardware at C5, C6, C7. There are interval discs spacers at C5-6 and C6-7. Uncovertebral joint hypertrophy causing mild RIGHT neural foraminal narrowing at C4-5. Bilateral uncovertebral joint hypertrophy causing mild bilateral narrowing C5-6 and C5-7. Straightening of the normal cervical spine curvature which may be related to positioning or muscle spasm. No acute fracture or dislocations identified.

    IMPRESSION:
No evidence of acute intracranial process.
No evidence of cervical spine fracture.
Post surgical changes in the lower cervical spine as directed above. Mild neural foraminal narrowing at C4-5, C5-6, C6-7.

I am experiencing swollen right side of neck along tip of shoulder as well as the right arm pain that waxes and wanes throughout the day with weakness, pain and numbness. I am also post op from 3 previous shoulder surgeries whereas ive been released as not being a shoulder issue. Please advise of any concerns that would warrant surgery or pain management if possible. Im not sure if i need to mention but i did have an accident where i tried avoiding a fall down the stairs.

Thanks a million for all your help!
    Kind regards,

This radiological report does not address whether or not there is a solid fusion. There are radiologists who will not comment on fusion but I think that is not appropriate. I think they need to make a call regarding fusion status.

Can I assume that your three shoulder surgeries were on the right? If so, then a good physical examination will help to determine if your shoulder or your neck (or both) could be causing your right arm pain. A selective nerve root block might then be in order to determine which level is causing pain. A shoulder block might also be in order.

Dr. Corenman, thank you for all your time for answering my question!

You are correct in the 3 shoulder surgeries. I was told i had scar tissue that kept coming bacb but an Mri Arthogram ruled out its not the problem. Ive lost internal rotation about 1/2 and even reaching behind my back, it to the gluteal region for over 3 yrs now. My question is now that its been so long, is it permanently related to nerve damage? I had an xray of my cervical spine but i cant look up or tuck my chin in during the test. The surgery c5-7 was using peek cages and bone marrow fluid from my hip.

    Thank you!
    Kind regards,

You need to have a radiologist (or your surgeon or preferably both) comment on your fusion status. You might need flexion/extension films to help diagnose the fusion status. You might need a second opinion from a spine surgeon (an orthopedist who specializes in spine surgery) so he or she can tell the difference between the neck and shoulder (neurosurgeons has less expertise in shoulder disorders).
