Hi!9 months ago i started had pain in my right arm, weak grip, neck and shoulder pain! I went to psysical therapy which helped me with tge arm pain! I had MRI which shows on C5/C6 mild circumferential disc bulge with a central area of disc herniation/protrusion type! For the last 4-5 months the pain is horrible! I have arm pain, neck pain on the right side of the neck, shoulder(scapula) and some part of rib cage pain and headaches! The pain in the arm and scapula is a burning pain, light sensitivity to touch sometimes! Also pain above and under collarbone! The pain is constant, better when i am sleeping! I had cervical epidural injection which did the pain worst! I went to several doctors which they don’t know what can be, that the bulging disc is too minor! I had also MRI for shoulder, for thoracic spine, 2 nerve test, everything negative! The only person that insists that is from the disc, especially from Annulus F is my physical therapist! I read that MRI can miss Annulus tear! Do you think that this constant, excruciating, burning pain can be from Annulus tear? After some sessions of physical therapy, the pain and burning in my arm improve a bit! Thank you!

It sounds like you have a compressive radiculopathy, probably at C5-6 where your disc has a herniation (“mild circumferential disc bulge with a central area of disc herniation/protrusion type”). It is impossible to determine based upon the description provided how significant the protrusion is. If you have weakness and a negative nerve test (I assume an EMG/NCV), you might have a peripheral neuropathy also. See https://neckandback.com/conditions/nerve-entrapment-compression/ and https://neckandback.com/conditions/symptoms-of-cervical-nerve-injuries/ for the C6 nerve.

Thank you, Dr. Coleman! I don’t have weakness, just pain and burning! I forgot to mention that everything started after a emotional trauma! Also just for one week i had like a sciatic pain in my right leg and burning feet! What doctor do i have to see, because all that i saw don’t know what i have!

Emotional trauma without a physical component makes me think of a pain magnifier syndrome like reactive depression or fibromyalgia. “No weakness” would lead to a negative neurological finding on EMG/NCV which fits. Sciatic pain onset at the same time is not physically related to the neck based upon your current finding but would fit with a pain magnifier syndrome. Ask you physician if they would consider a pain-modifying medication like Gabapentin (Neurontin).

Thank you! If is nerve entrapment, what is the treatment? Because all doctors i saw they don’t know and they said they don’t see nerve compression! Also the rheumatologist said that doesn’t sound fibromyalgia, that is more nerves but doesn’t know where from! I really don’t know where to go anymore!
