You need to list your symptoms before and after the surgery in chronological order, why you had the surgery (what were the expectations for relief) and what happened after the surgery. List this with dates on the left and symptoms on the right, what specific surgery was performed and how your symptoms changed. I know you are caught up in this emotionally, but you need to be level-headed to describe your symptoms so I can know where to go with this.
