    I had DDD and a pain on my left side at L5-S1. In 07 I had a Charte Disc replacement. It took care of my disc pain but the pain on the left side got worse. Doctors thought it was SI pain. After 5 years of injections I decide to have SI fixation. The pain moved closer to the center but still on the left side. Doctors thought it was Facet pain so I decide to have them removed and just fuse the L5-S1 level. I still have the pain on the left side. My question is was could be a pain generator at L5 S1 if it is not the Disc, SI or Facet? It has all of the makings of Facet pain.It is tender to the touch Twisting and bending make it worse. Standing still makes it worse, and I get relief from changing positions.

    An artificial disc replacement in the lumbar spine generally does not help with nerve generated pain and most SI pain is either nerve or disc generated pain.

    If you had a total facetectomy bilaterally (on both sides) and then a posterior instrumented fusion but left the artificial disc in place, I would assume that you most likely have a pseudoarthrosis. Did you have a CT scan after this fusion to demonstrate a solid fusion?

    The way to diagnose the origins of pain is with nerve and facet blocks (see pain diary to understand this concept) or discograms.

    Hello, thanks for the reply. The pain in between the Facet and the SI. CT indicates fusion fine. Only other thing is MRI indicates mild L4-5 degeneration and Facet hypertrophy. However, the pain is felt at L5-S1.

    Back pain generators are one of the great conundrums of diagnosis. The location of pain many times does not correlate with the structure that causes the pain.

    Are you sure that you have a solid fusion at L5-S1? With an artificial disc in the front and full facet removal in the back, this is the first of the differential diagnoses that I suspect. If you are certain that you have a solid fusion, I’ll move on.

    Pain in the lower back that is off to one side certainly can be generated by facet, nerve or disc. Rarely can the sacroiliac joint cause this pain.

    Start with blocks of the facets and the nerves (see website for this information). If there is a reproducible block that temporarily relieves pain (using the parameters of the pain diary-see website), this most likely is your pain generator. Treatment will depend upon the structure that is involved.

    There are occasions that this pain could be from chronic radiculopathy (see website) and no surgical intervention will be helpful. This is a diagnosis of exclusion.

    Could the pain generator be the Facets at L4 but felt at L5-S1? The MRI indicates hypertrophy at L-4 but the pain is lower.

    Pain origination could be from the facets at L4-5. It might be worth it to have facet blocks at L4-5 to see if you get temporary relief from these blocks.
