    Your C5-6 level is fused so it is stable. This C5-6 fusion does nothing for C4-5 except to slightly increase the risk due to the inability of C5-6 to “cushion” the C4-5 level. The C4-5 level is somewhat unstable but there is no dividing line between what is stable and what is unstable. The real definition of instability is the inability to withstand neck forces that would put the cord or nerves in jeopardy. Therefore, instability depends upon the forces that you subject your neck to. Your neck could obviously withstand a pillow fight but not a direct impact with body with forces.

    Hi Dr. Corenman,
    I just wanted to Thank You for viewing my X-RAYS and answering my questions for me, I really appreciate it. Dr.Corenman I know my neck is in bad shape,but I bet you have seen worst.

    Dr.Corenman, You viewed my X-RAYS do I have a lot of osteoarthritis in my cervical spine that could be causing the aching pain I been having in the back of my neck and that goes up into the back of my head , especially when it’s cold and damp or raining outside or when I’m in an air conditioning room.Dr.Corenman or could the chronic Ligamentous Instability that I have at C4-C5 level be causing the aching pain or combination of both the arthritis and the instability?? Is the Ligaments at C4-C5 level mildly or moderate damaged? That could be causing the aching pain.

    Hi Dr. Corenman,
    What Grade of Spondylolisthesis do I have at C4-5 ? Grade 1 or Grade 2

    Dr.Corenman,Can you tell me from viewing my X-RAYS,did I actually tear my Ligaments away from my vertebraes at C4-5 level ? Like my Spine Doctor and the Chiropractor said I did. They both told me that I’m lucky to be alive with the unstable neck injury I had. Could you please let me know how my Ligaments at C4-5 level looks to you ?

    The Chiropractor told me that I have Chronic Ligamentous Instability at my C4-5 level, because my Ligaments are damaged at C4-5 level. I would like your opinion because you know your stuff when it comes to cervical spines. Could you please let me know if I have Chronic Ligamentous Instability at C4-5 level and if the Ligaments are mild or moderate damage ? Could this be causing my pain that I been having in my back of my neck that radiates up into my back of head.

    Neck pain can be generated by disc degeneration, facet disease, malalignment, instability or nerve compression. You have may different potential causes of pain and would need a workup to determine which one (or ones) would be causing your pain. This would include facet blocks, nerve blocks and even possible discograms. Sometimes a thorough history or the physical examination can lead to a more specific diagnosis and narrow down the potential pain generators.

    Your slip a C4-5 is mainly caused by facet joint degeneration. The ligaments are secondary restraints and are a problem but ligament laxity or failure would by itself not allow the slip to progress. Yes, the degenerative spondylolisthesis (slip) at C4-5 could be causing some or all of your symptoms.

    In general, slips in the cervical spine are graded on millimeters of motion and not grade. Grade is limited to the lumbar spine as most slips in the neck would not be greater than a grade 1 due to the anatomy.
