    hi Dr,
I had a c6-7 cervical fusion in nov last year, and a decompression in march this year,
there were some minor issues with c3/4 then,
in july I hit my head on a scaffold board (with a hard hat on) it was quite a hard hit,
ive had some burning nerve type pain in my neck and shoulders, ive been going to physio with no luck,
my dr sent me for an mri on sunday and I got a copy of the report today for c3/4 it had the following
osteophytosis, bilateral facet arthropathy, moderate bilateral foraminal narrowing, disc desiccation,
moderate bileratal uncovertebral spurring, , impingement of exiting c4 nerve root sheaths,
im just trying to get some idea of what im dealing with before I go back and see my surgeon,

    thanks

I am unclear if you had a C6-7 fusion and then a decompression posteriorly at the same level sometime later or a decompression at an entirely different level.

Burning type nerve pain that radiates into the neck can originate from compression of the C4 nerve (nerve originates from the C3-4 level) or from discal or facet generated pain at the same level. You do have “moderate bilateral foraminal narrowing, disc desiccation, moderate bileratal uncovertebral spurring” which typically can also have associated degenerate facet disease.

First treatment is to undergo physical therapy or chiropractic with some medications thrown in. If you have no acceptable pain reduction, possibly the next step is to consider some diagnostic injections (facet or SNRB)- see https://neckandback.com/treatments/epidural-injections-and-selective-nerve-root-blocks-diagnostic-and-therapeutic-neck/ and https://neckandback.com/treatments/facet-blocks-and-rhizotomies-neck/ and https://neckandback.com/treatments/pain-diary-instructions-for-spinal-injections-neck/ and https://neckandback.com/treatments/diagnostic-therapeutic-neck/

    thanks for your reply dr,
I had the acdf of c6/7 in November, and then a posterior decompression of c6/7 (due to bone spurs) in march,
ive already been having physical therapy for this current issue with no luck to date, I did have a nrsi injection at c6/7 which didn’t help, but I am open minded, I will make an appointment to see my specialist and see what he would recommend, hes very conservative and does not jump straight into surgery without trying other options first

“I did have a nrsi injection at c6/7 which didn’t help” which is understandable when your problem more likely is at C3-4. An injection at C3-4 would be more helpful to diagnose this level as the pain generator. If you have failed conservative care as it seems you did, that could be the next step in my opinion.

the injection was prior to the c6/7 decompression, I will talk to my specialist and see what he recommends
I will update on here what happens,
