Hello Dr.Corenman, I have a question WHAT IS CHRONIC LIGAMENTOUS LAXITY OF C4-C5 MEANS ?? Does this mean it’s unstable at this level ? Could you please explain!! Thank You !!

Chronic ligamentous laxity generally refers to instability of this level due to both ligament, discal and bony changes. The stability of the cervical spine is maintained by the disc, facets and ligaments. The disc is the shock absorber and will give some (but not precise) stability to the spine. The facets are what I call “railroad tracks” and are important stabilizers for fore and aft motion of the vertebra. The ligaments also prevent motion (translation) and are important stabilizers. See https://neckandback.com/anatomy/anatomy-of-the-cervical-spine/, https://neckandback.com/anatomy/normal-spinal-alignment-neck/ and https://neckandback.com/conditions/stabilizing-ligaments-upper-cervical-spine-occiput-c2/ to better understand this concept.

Hi Dr.Corenman, Thank You for answering my question so promptly !! I had a neck injury back in 1988, when I fell down a flight of stairs and torn my ligaments away from my vertebraes at C4-C5 Level and had a compression fracture at C5-C6 level and had to wear a halo brace for Three months. Now in the present the one Doctor told me That I have a auto fusion at C5-C6 level That helps stabilize my neck at C4-C5 Level, while the other Doctor I’ve seen told me that the auto fusion was nothing but severe arthritis and that I still have instability at my C4-C5 Level and if I ever was in car accident or a bad fall that the Auto fusion would not support my neck.Which Doctor do you feel is correct because I’m very confused. Thank You !!

Be careful who you take advice from regarding X-ray interpretation. I have many patients who come into my office discussing their “auto-fusion” where they were told by a non-spine professional that they have a fusion. What is really present is severe degenerative disc disease (called “IDR”) and endplate abutment (the disc degenerated so thoroughly that the bony endplates of the vertebra are banging into each other).

Now-you can develop an auto-fusion but being pessismitic, I would assume that you have IDR. ALso, even if you have an auto-fusion at C5-6, this will not stabilize C4-5.

Hi Dr. Corenman, I was wondering if could please view my Flexion and Extension X-Rays that I have on my Email account and then let me know if I still have a mild to moderate instability at my C4-C5 level from the neck injury I had in 1988. Dr.Corenman the Spine Doctor who treated me in 1988 told me that I would have mild to moderate instability at C4-C5 level for the rest of my life. I would be greatly appreciated if you could please view my x-Rays and give me your opinion on the instability that I have at C4-C5 level. Dr. Corenman if you would I would need your Email address so I could Email them to you to view. Thank You

If you have already been a patient in the clinic (even from 30 years ago when I was at the University), you are welcome to contact my manager, Lori Fugate to have me review your films.
