    Thanks for the reply. So after 2 Transforiminal ESI injections, Intense pain (standing/walking) has been some what manageable by taking 1 100mg Nycynta , 2 Zanaflex and 1 Percocet 5/325 at night. My biggest problem is walking and standing for more than 10 minute and the pain is better in the morning and gets intense by evening. The left butt pain would start and eventually spread to leg and tingling/little numbness in the left toe.This is my effecting quality of life.
    I had 1 facet injection on January 3rd and I can’t really feel any difference .Will medial branch block injection help? Or should i just ride it out with medication and when it gets really intense got for Transforiminal ESI ?
    I am trying to figure out what are my options beside a spine cord simulator which my pain doctor seems to be more interested .Again, My biggest problem is walking and standing for more than 10 minute.

    Dr. Corenman-So i had 2 Transforiminal ESI injections,one on October 7th and one on November 2nd. Pain is back up again, went to see pain management Doctor. He doesn’t want to do anymore ESI since I had 3 of them before the surgery back in last December. I asked him what my options are, he said, may be spine simulator. I am not sure why he din’t told me about Radio Frequency Neurotomy, so I asked him whether RF would help. He said before RF , he needs to do 2 facet injection. So I am scheduled for facet injection on January 3rd. My question is why would the pain doctor would go directly to spine simulator without trying anything else. Any thoughts? I thought spine simulator was the end of line option.

    Thanks for your reply. I have taken 2 Transforiminal ESI injections. Pain is no longer intense but still there. Guess my only option now is to wait.

    Thanks Dr. Corenman.I think the radiologist made it short since nothing much was changed since last MRI w/o contrast. I summarized the MRI with and without contrast for you below.
    MRI w/o contrast: At L4-5 there is evidence of left hemilaminotomy with postoperative change. Soft tissue is
    seen within the left lateral recess is unclear as to whether this represents postoperative
    change or disc material. There is apparent mass effect upon the left L5 nerve root.
    Impression:L4-5 postoperative change with question disc herniation versus granulation tissue.”

    MRI with contrast: Examination demonstrates at L4-5 once again left-sided postoperative change. There is mass
    effect within the left lateral recess which has decreased as compared to prior examination
    and demonstrates contrast enhancement consistent with postoperative change.
    Impression: Postoperative change. No herniation noted.

    If I understand correctly, he did said, there were no herniation. My NS also concurred that after reading MRI but it could be scar tissue and i’ll eventually get better over time. Any suggestion on what do next ?
