You certainly might have a degenerative spondylolisthesis at L4-5 based upon the degenerative nature of the disc and the quote “.BILATERAL facetal anthropathy and ligamentum flavum hypertrophy is appreciated at this level”. See the website for a further description.

I would think that a set of standing X-rays with flexion/extension views would be helpful.
