    Dr. Corenman,
    How soon can one see the start of bone fusion after ACDF surgery from an x-ray?
    At what point in time, does it become a concern that an x-ray is not showing fusion?
    What are your thoughts about external bone stimulators to help with fusion?

    It really depends upon the quality of the carpentry. The line between the graft and the host bone (vertebra) should disappear to indicate fusion. This becomes much more difficult to assess if the surgeon uses PEEK cages (plastic). Even with a solid fusion, these cages can still look unfused. This is when the physician can use flexion-extension X-rays. The gold standard is CT scan.

    I would like to see fusion in an autograft case to fuse in 6 weeks. In an allograft case (donor bone) to fuse in three months. I have seen some of these cases fuse in six months.

    External bone stimulators may help in a fraction of cases to help fusion occur but I think they are not worth the time or cost.

    Thank you for your reply. Just to clarify…
    With donor bone used and titanium plating, would one expect to see some indication of fusing at 8 weeks or is this too early to see anything on an x-ray?

    With donor bone (an iliac crest donor graft and not moralized donor bone in a cage), the donor bone-host bone X-ray line can linger for about 2-3 months. I think this is the time that the fusion takes place.
