    At C3-C4, there is mild left facet hypertrophy and bilateral uncinate spurs. No spinal stenosis. There is mild bilateral neuroforaminal narrowing greater on the left.

    At C4-C5, there is minimal posterior disc bulge. Moderate left uncinate hypertrophy is seen. There is moderate left facet hypertrophy. There is no spinal stenosis or right neural foraminal narrowing. Mild left neural foraminal narrowing seen.

    At C5-C6, there is mild disc bulging and tiny uncinate spurs. Moderate to severe right facet hypertrophy is seen. There is no spinal stenosis or left neural foraminal narrowing. Mild right neural foraminal narrowing seen.

    At C6-C7, there is mild posterior disc bulge. No significant spinal stenosis or neural foraminal narrowing.

    At C7-T1, no significant findings.

    Impression
    IMPRESSION:
    1. Mild to moderate cervical spondylosis identified at C4-C5 and C5-C6 as detailed.
    2. Mild neural foraminal narrowing seen in the mid cervical spine.
    3. Cervical cord has normal appearance. No spinal stenosis.

    My pain is burning in my shoulders and stiffness in my neck resulting in inserting more migraines. I also have extreme pain in my hips and lower back. This comes on anytime I do any repetitive work such as lifting and driving. This has been worsening over a period of 10 years

    My question to you is am I looking at surgery due to the fact physical therapy muscle relaxers and exercise give memo relief and I seem to be getting more continuous pain
