    Hello Dr,

    A patient is consulting to you with the below complains.

    1.major complains
    -when I walks I feel like I might fall after some steps in between.feeling hand and leg light weight.
    with movement of the neck (looking up),feel more of these symptoms.
    -have to do more efforts while standing and sitting.
    -other complains
    -some times a more blur vision,dizzyness.
    – cannot hold neck properly, feeling stiffness.

    After assessing the patient what are the diagnostic test you would suggest.

    The symptoms could be related to cord compression and myelopathy. An MRI and X-rays would be in order if the physical examination points to this diagnosis.

    patient with complain.

    -during walking when I see straight or look up I lost the control of my hands and legs and feeling almost fall down.when I look down I feel ok with these symptoms.

    blurry vision.dizziness.

    After assessing the patient what are the diagnostic test you would suggest.

    The symptoms could be from cord compression or vertebral artery compression. The diagnostic tests for cord compression include a physical examination for long track sign and an MRI of the cervical spine. The test for vertebral artery insufficiency is a CT angiogram with rotational maneuvers of the head (see Bowhunters syndrome on this website).

    when Doctor suspects ?vbi vertebral artery insufficiency how to confirm that?

    is it possible to determine percentage of insufficiency ?

    The only test that I know is accurate is the CT angiogram with head positioning in the position of symptoms. The dye is then injected into the vein and the CT scan will indicate if the blood flow of the vertebral artery is “cut off” with the head in the position that causes symptoms.
