    Hi Dr. Corenman:

    Have you ever heard of this and can it be used as an alternative to BMP in TLIF ?

    I have not used this product. It contains the typical osteoconductive matrix (a scaffolding that “welcomes host bone cells” to migrate in from surrounding bone). The second is an osteoinductive matrix (supposedly having something “similar” to BMP that will cause your stem cells to turn into bone cells). Finally the contents have MSCs (mesenchymal stem cells) that are viable and can turn into bone cells.

    The last component makes me worried. How these stem cells stay viable is one question as with any living tissue, these cells should have a very short shelf life. The second and more concerning question is what can happen with implantation of cells into your body with a different DNA than you have. Could there be some host/graft immunological reaction or could there be some change in these cells down the road that could be catastrophic to you?

    I have concerns the would make me very apprehensive using this material.

    Is the Trinity Elite Allograft the same thing as DBX ?

    If not, what is the difference ?

    My surgeon would like to use either or in place of BMP for MIS TLIF.

    The Trinity Elite Allograft is a combination of scaffold, reported proteins that are “expected” to mimic BMP and viable human cells. That third one worries me. These are cells that are not DNA comparable with your host cells. I assume that these cells are treated to reduce their host-immune system response. I am not comfortable at this point recommending this type of cell implantation as the potential risks have not yet been fully assessed.

    So Demineralized Bone Matrix is not the same as Trinity elite ?

    Is DBX as potent and effective as BMP ? Surgeon wants to use instead of BMP…

    Demineralized Bone Matrix is not the same as BMP. DBM is bone matrix without the hard calcium. Calcium is a mineral and “demineralized” simply means that the hard crystalized calcium has been removed. This leaves the protein and collagen background that is malleable can be “moulded” into whatever defect that needs to be filled. This material does have some natural BMP within it but not quite enough in my opinion to really “turn on” the bone repair process.

    BMP is really the best substance (other than autograph or your own “ground up” bone) to create bone formation. If you don’t have either, then DBM is acceptable as an alternative source.
