    Thank you for this amazing educational service you provide!
    I am trying to find effective information regarding a nearly 4 year-old skating injury: landing hard on my right flank while pulling a counter-rotational twist. I have seen a number of practitioners however my morphing symptoms continue. The pain began a week or 2 after the fall, starting with abdominal and progressing to involve my flank and ribs. It became severe, was aggravated by breathing or any activity. It took awhile to realize that although it felt visceral, it was probably by back.
    I have had 2 MRIs, and the assortment of degenerative changes has gone from mild to moderate over the 2.5 years between them. The latest “confirmed” a right T9-T10 radiculopathy (“paracentral disc protrusion resulting in mild canal stenosis…costovertebral spurring with encroachment on neuroforamina”, etc). I was offered injections. My lumbar spine is also kind of bad, including a L4-L5 herniation. I saw a neurosurgeon 6 months prior to this latest MRI, and he said that I “tore a bunch of muscles..like whiplash…and you’re on your own with that…”
    A PT perfomed several sessions of MFR, and identified involvement of the quadratus. I also have a vertical area slightly above the QL which if I press into while flexed forward feels like a sensitive wad of scar tissue. I get temporary relief by rolling it out but it comes right back. Ditto for stretching and somatics/yoga, and TENS. I have developed crunching, grinding, and popping of my spine (mostly right). I still have occasional sharp pains in my RLQ and pulling around the iliac crest. Lots of back spasms, spine feels sharp and crusty.
    I do not want to go though injections if the muscles are the cause. But all the doctors said “no, muscles cannot be imaged”. I find evidence to the contrary, including PTs that do realtime US of multifidi to evaluate muscle engagement. A CT and IVP (yes, I had a kidney stone – ha ha)showed a benign calcification lateral to the kidney which was interpreted as “arthritis on the rib”. I am concerned that is an indication of torn muscles or tendons. Also, I wonder about ligament tearing. Please, if you have time, what are your impressions of all this? Do you think cold laser is effective?

    Muscles can be imaged on an MRI but it is unusual that muscles tears would cause your symptoms. Injections are not just for treatment but are very valuable to diagnose conditions. If the thoracic disc hernation is causing your symptoms, a selective nerve root block will give you temporary relief of your symptoms. See “pain diary” to understand this concept.
