    Generally, tenderness is only important if associated with pain in the facet regions. Many individuals have tenderness without any pathology or spinal disorder.

    The pain is actually spreading to the surrounding area on the left side. MRI failed to show anything. Any ideas as to what it could be? Just to recap, the spinous process is sore only on the left side and the pain actually spreads to the left area. Thanks.

    Local pain without obvious mechanical source visualized on MRI/CT can be caused by neuropathy-microscopic damage to the pain nerves (nociceptors). There are some disorders that are inflammatory in nature such as Polymyalgia Rheumatica- a disorder of muscles. If it is not a mechanical or inflammatory disorder, some individuals would call this myofascial pain syndrome and some call this condition fibromyalgia. Treatment is difficult if this is the case. Injections, physical therapy, chiropractic treatment, TENS units and finally possible implanted spinal or peripheral stimulators.
