Hi Dr. Corenman. For the medical records for the long distance second opinion service do you want surgery reports only or surgery reports as well as clinical notes?

Surgical operative reports are helpful but most of the time unnecessary as the post-operative MRI and X-rays (CT also sometimes) are generally enough to tell the story of the surgery and the aftermath of such. The main important items are actual values (lab tests, EMG/NCV studies and all images). That information along with your actual history and telephone interview are sufficient to understand the disorder.

Thank you Dr. Corenman. I’m working with my PM doc to get updated imaging. Once complete I’ll get this setup.

My PM just put in the order for a fresh CT scan, and my most recent MRI (with contrast) was on June 12. Once I get the CT completed I’ll get the second opinion request setup with you.

Make sure the CT is on a 64 slice or greater scanner (128 or 256) with 1mm cuts and sagittal/coronal reconstructions.

Thank you doc. Waiting on them to call for scheduling, but I’m going to call over to the radiology department where I’ll be having it done and ensure they can meet those specs.
