    Hi Dr. Corenman,

    I’m almost to the 7-month mark post op from my fusion and have developed a new, significant pain that is becoming most worrisome.

    My CT scan at 5 months showed significant signs of fusion (solid columns of bone between end plates) and I was given permission to get back to normal life and begin increasing exercise at the rate or 10-15% per week. I have done this meticulously until a week ago. I have only been riding my bike, mountain, and road, and have not crashed or had any hard hits, trauma, etc. I have also been following a stretching and resistance program (elastic bands and body weight only) to increase strength in my back, glutes, and hips.

    Last Wednesday I developed a very sharp pain in my sacrum area off to the right side. I don’t have any triggers or localized swelling or pain that I can push on, however, when I do a cat pose (arch my back and tuck my tailbone under) I get a VERY sharp pain! I had this few a weeks or so back in June, and it went away after I did some stretching. I was told it might have been breaking through some adhesion/scar tissue. I thought it was the same thing this time (nearly the same exact pain), so I followed the same protocol and stretched through it in hopes it would get better. Its only gotten worse. This started after a mountain bike ride on Wednesday last week, which was uneventful. After the ride, I was stretching and felt the sharp pain return and I was able to push through it. The pain would subside and I could increase my ROM before the pain would return. Each morning the pain would increase and my ROM was significantly decreased from my previous progress.

    I have the pain when sitting and shifting my weight ever so slightly in a chair. I have backed off all exercise and stretching as it causes pain. I’m back on Aleve 2x tabs, 2x daily and heat, ice and using my tens unit again. It doesn’t seem to be getting any better.

    My fusion was L5S1 and the pain feels centered around the base of the sacrum/coccyx area about 4-5 inches below my incision site. I can’t seem to locate any sharp pain when pushing on this area, only when moving my hips.

    Is this a wait and see scenario? Any concern about hardware loosening or breaking? I only ask because nothing I have done seems to have set this off.

    You have a unilateral pain after a fusion 7 months ago at L5-S1. The differential is pseudoarthrosis, new level involvement (L4-5 or possibly the SI joint). a nerve compression syndrome at the fusion level (heterotopic bone formation or seroma/adhesions). If it is tolerable, give it some time. If it is not tolerable, a new CT or MRI would be one option, a short course of steroids another.
