    Hello Dr Corenman. I am in need of your opinion.

    I wrote before. I had l5/S1 micrdiscetomy on 7/20/21. I was doing great, no pain. Bad Flare up at 6 weeks after bike ride-spin. I had pain up to 8/10. Surgeon ordered MRI and steroid pack and said it looked okay just scar tissue. Pain decreased and Took a few weeks off and modified work. Had some lingering pain and stiffness. Saw a pain MD who recommended nerve blocks and then ablation because my facet joints looked inflamed and misaligned. The median branch blocks seemed to be working at two week intervals. I remember feeling better. I was slowly getting back into therapy and doing great. Pain 1-2/10. More light spin riding and started light yoga. The first ablation on the left side (which is the worse side) seemed to work but then started to develop more pain on my right side lumbar area. I was still able to work after first ablation and light work out weights/therapy and after the second ablation I developed severe stiffness and severe pain along my facet joints and muscles in lumbar area after about 1 week. I ended up in ER after I could not get up from ground and rotate. Was able to drive to hospital. The MRI said more inflammation and my surgeon said inflammation and nothing urgent recommended epidural and different pain MD. I am off work now able to walk but very gingerly with pain up 6-8/10 in back brace. I am just very careful and worries.

    Questions- do you think there is permanent damage from ablation? I am out of work now. I am walking but cannot really bend or twist without feeling like my back is “going out” all this pain on my right lower side. I walk supporting my right lower back. Slightly away from midline if I could describe location

    Will this heal over time? Do you think any more injections can help or hurt? Facet or epidural would help? I greatly appreciate you time.

    MRI report. L5/S1 Desiccated disc without significant prolapse. 6×10 mm area of increased T2 signal along the left posterior margin of the l5 s1 extending into the left lateral recess. No additional evidence of herniated lumbar disc or of lumbar central stenosis

    Status post l5 S1 micro Lami And post surgical edema on paraspinal tissues

    All of my pain seems to be facetogenic where I put my thumb in lower back. I have a hard time extending and get relief with flex ion. Hard to lie down pain can get bad and then I feel my lower back freeze. Please tell me this will subside over time or about a facet injection or anything else that will help. Thank you so much for you time.

    I have an update. Pain is very bad in mornings. Very localized along right lower side near L/5 S1 but more lateral. Sometimes I am limping. Deep throbbing very painful trying to extend my back and lay prone. I keep pin pointing it right above PSIS. I have pretty severe groin pain as well.

    Surgeon thought maybe SI related. They did an SI joint injection today. New pain doc from my surgeons practice recommended this pain doc. I still feel a lot of pain it seems like maybe it was facet related. I know it can take time for injection to work. Thank you.
