    Hello!

    I wrote you about six months ago and you were kind enough to give me some information.

    Recap: I had fusion from c5-c7 back in January 2015. My left arm was very painful after surgery (when I woke up) and it continued to get worst after I was already in PT. First EMG/NCV showed no nerve damage. I kept pushing for another one, because I couldn’t extend it at the elbow, I couldn’t lift it on my own higher than my waist, it shook, hurt a lot, etc. So, they did another EMG/NCV and it showed chronic nerve damage (chronic denervation in bicep and lower part of arm). I tried 4 more months of therapy with no improvement. My insurance wouldn’t pay for any more therapy, so either way I was done with it.

    It is very weak. The muscle, even when I am shopping, folding laundry, driving will get tired quickly and shake (kind of how your legs shake after a long hike or run). I still can’t extend it nor lift it properly. I have twitches and shaking. I have probably lost, just by my estimation, 40% of the use of it. I got a second opinion, but all he could offer was more meds (serotonin type meds) and said to keep working on the muscle. But he did say, likely I wouldn’t get too much use back. No one wants to say this is as good as it gets.

    My neck and arm hurt all the time, especially when waking. Of course with the attention on opiods, getting medications is awful. I am on Tylenol 3 (one pill every 8 hours, which doesn’t help. I take regular Tylenol with it), Gabapentin, and Baclofen. I am severely allergic to all NSAIDS-last time I took them I had huge football welts all over my body and throat was closing. My doctor is trying to get me a new CT scan just to make sure there isn’t anything going on with the fusion area-rule out everything. The last MRI didn’t have a clear view of the c6-c7.

    So, my question is: Would a nerve block help at this point? He hasn’t offered and honestly I don’t even know if I can afford it out of pocket. I am just wondering if this is as good as it will get. I lost my job, because I was out of the office so long after surgery. Even just typing this is tough. I wear a brace that I bought, for long walks, shopping that takes longer than a few hours or if it is hurting a lot. The doctor said to not use it a lot or my muscle will atrophy more. But, honestly sometimes I can’t take it. I still have some herniations in c4-c/5 but he doesn’t think this is causing the problem.

    Significant weakness certainly could be from nerve root injury and a CT scan might be helpful. I like nerve blocks for two reasons. The first is diagnosis. If the C7 nerve is involved (which it sound like due to your lack of ability to extend it-triceps muscle weakness), a block would confirm that the nerve is the problem (see pain diary). In addition, steroid is injected and can be beneficial for the nerve root.

    I hope someone has carefully looked you over for shoulder problems (rotator cuff tear).
