A referral can be for a second opinion if the specialist who gives the referral is of like kind. For example, if a neurosurgeon sends a spine case to a spine surgeon for an opinion, this would be a second opinion.

In your case, the neurosurgeon is referring to an orthopaedic surgeon for an ankle disorder. This is a “consultation” as the neurosurgeon is not an expert in ankles and this particular ortho surgeon is probably not an expert in spine.

What can you expect from the orthopaedic surgeon? Hopefully, he or she takes a thorough history, a complete physical examination and reviews all salient images. The differential diagnosis is made, any confirmatory tests are completed and treatment is ensued.
