    For a long distance consultation, you would call Lori Fugate, my office manager at the office number (970) 476-1100. You would need to gather all your images on DVD/CD (very common media) or a thumb drive and Fedex them to me along with any prior consultations and diagnostic injections if performed. In addition, there is a 7 page introduction history form that you would have to fill out and either email or snail mail. After these all arrive at my office, I review the images and call you. We generally spend about 20-40 minutes on the phone and any follow-up conversations are included. I dictate a report and send you a copy. Remember that I cannot prescribe anything but give you direction for your disorder. The fee I am not sure of so you would have to talk to Lori about that.

    Thank you I will contact your manager and will gather all reports and FedEx them. Also is it possible to schedule consultation on weekend like Saturday? I work 10-13 hrs a day and also I would like my husband to be present during conversation because he speaks fluent english

    I generally call late during the weekdays as I too work 10-16 hours/day. I don’t make calls on weekends generally or my wife would divorce me and kids disown me (they have enough problem with the forum on the weekends!)
