    Your symptoms have to be carefully sorted out and then diagnostic blocks should confirm diagnostic suspicions. Revision surgery can be quite effective but I can’t determine if it will help you with the current amount of information.

    I have uploaded the pictures that shows the non fusion.

    I have done a nerve block and it help the scalenes, they become more loose and I could move my head from side to side more smoother.
    But the stiffness in the levator did not help.

    If I do a posterior how long is the incision. I’m afraid of more stiffness in my muscles.
    And from the pictures, do you think I need to stable c3 C4 and remove the pseduarhrosis with a new surgery.

    With that much motion (13mm change) between the spinous processes and significant neck pain, you probably need to have that C3-4 level fused. Posterior fusion is a good way to go for this problem. Depending upon the surgeon, the incision should be about 2-3 inches, Fixation can be performed with rods and screws or a cable fixation.

    I have now received a possible date for the posterior fusion and will have a appointment next week for final consultation.
    Do you think it’s wise to fuse from c3-c6 although c4-c6 is fused with < 1-2mm movement or its enough with c3-c4

    You probably don’t need C4-6 addressed posteriorly but there is really no penalty for including those levels already operated on. In the unusual situation that one of those levels is a stable pseudoarthrosis, it would be worth it to have all levels fixed.
