    Hi from Down Under,

    I am 59 year old male weighing 70KGs (156Lbs) who underwent surgery on 29Aug16 of the lumbar spine with decompression laminectomy at L4 (partial) and L5 (full). The surgery was to rectify moderate foraminal stenosis for the exiting L5 nerve roots.

    On 31Oct16 I had a follow up MRI of the Lumbar Spine which reflected the following:

    Post OP unenhancing granulation tissue in the posterior para-vertebral space and also within the central canal surrounding the thecal sac at L4/5 and L5/S1 is noted.

    There is mild S-shaped scoliosis of the thoracic spine and attenuation of the lumbar lordosis.

    There is slight retrolisthesis of L3 on L4 and L4 on L5, otherwise there is preservation of posterior alignment of the remainder of the lumbar spine.

    There is mild wedging of the T12, L1, L2 and L3 vertebral bodies and the greatest reduction in vertebral body height noted at T12 of approx 20%.

    The conus terminates at the level of T12 and is unremarkable.

    L1/2 – The L1 nerves exit satisfactorily.
    L2/3 – The L2 nerves exit satisfactorily.
    L3/4 – Mild canal stenosis.The L3 nerves exit satisfactorily.
    L4/5 – Broad based disc bulge impinging on the anterior aspect of the thecal sac. The L4 nerves exit satisfactorily.
    L5/S1 – Posterocentral disc bulge impinging on the anterior aspect of the thecal sac. The L5 exiting foramina are clear and the nerves exit satisfactorily. The S1 nerves exit unremarkably.

    There are mild degenerative changes to the sacroiliac joints bilaterally seen.

    Radiologist comments:

    Mild spondylosis at multiple levels and canal stenosis of variable extent at several levels is seen.

    Post OP changes in the paraspinal disc space at L4/5 and L5/S1 but no significant nerve root compression is seen.

    No significant disc protrusion recurrence is demonstrated.

    What I would like to know is whether in your opinion:

    a. whether the pain that I am still suffering (albeit to a lesser degree pre OP) which goes down my right leg to the large toe could be expected to disappear over time.
    b. is there anything identified in the report which gives you any concern, and if so what course of action would you recommend that I take.
    c. in time could I expect further pain as a result of the disc bulging.
