    Dear Dr. Corenman,

Would you please comment on my MRI results:

Apr 2017: “Moderate uniform L5-S1 disc space loss. Moderate to severe broad-based posterior disc bulge with a superimposed left paracentral disc herniation. Measures 16 mm at the base and 6 mm in AP dimension. Severe obliteration of the left lateral recess. Severe right neural foraminal narrowing. Severe left neural foraminal narrowing. Moderate central spinal stenosis. Displacement of both the descending S1 nerve roots, left worse than right.”

Feb 2018 second MRI results: “Advanced (significant) DDD at L5-S1 with disc height loss, endplate osteophytosis, probably some associated endplate edema, mild hypertrophy of the facets, Disc osteophyte complex with central disc herniation and mild central stenosis, abutment of the traversing S1 nerve roots however without significant displacement of them. Overall improvement from the prior MRI which had a left paracentral herniation. Moderately severe stenosis of the foramina which may irritate L5 nerve roots. Mild disc bulging at L4-5.”

Nov 2018 third MRI results: I don’t have the details but the radiologist said that It shows a significant and progressive L5-S1 disc height loss and the same herniation from the previous MRIs looks smaller this time on the same side but larger on the opposite side (front side of the disc). In addition, disc bulging at L4-5.

    Symptoms
Last 2 weeks: Intermittent unbearable debilitating pain in the lower part below the waist. Specifically in the pelvic perineal area, inner side of both buttocks around the sacrum and both thighs. Both legs very weak and tired from constantly tight tighs, buttocks and calves. Tender if pressed harder. Accompanied by uncomfortable pain and feeling full in rectal area and intermitent low back pain. In addition, occasional pins and needles around the scrotum, perineum and tighs.

Standing or sitting for longer than 10 min becomes difficult. I get sudden weakness and pain. Lying doesn’t help much in any position. When lying on either side the weakness and the unbearable dull electric tingling kicks inn that barely gets better by changing position. Walking feels better, but legs quickly get weak and tired. The feeling gives me shakes. Strong pain killers don’t help much.

Previously: All of the above, intermittent for about 6-9 months but with much lower intensity and frequency. Bearable without pain killers. Before that the pain on daily basis only in the low back area going down one leg usually after some physical activity.

Thank you very much for your help!
