    Hello Doctor,

    I have a question regarding what I have been diagnosed with. My MRI shows the following:
    1) L4/L5 Mild Protrusion could be touching the Nerve root
    2) L3 Mild Disc Bulge
    3) Spinal Stenosis (Mild to Moderate on L4/L5)
    4) Spinal Stenosis (Mild)
    5) Degenerative Disease (Which I know comes with aging)

    I’ve been diagnosed with these since October 2015 and I am doing the following therapy since January – Spinal Decompression, Chiropractic Traction care and just started physical therapy. My question is that my pain has gone down a lot but I have too many frequent episodes. Two of the Neurosurgeon told me that I needed a spinal fusion which I am trying to avoid. I am getting better but I lose hope when I get the flare up.

    Will this heal naturally or should I start consulting for surgery? I’m taking Ibuprofen every 3 days when it flares up. I put ice on it every night and take hot showers in the morning. I sleep on the floor. I wish I can be normal again but doesn’t seem like I can be EVER normal again. Any advise would be appreciated.

    Healing is a yes and no response. If you have read about degenerative disc disease, you will see that the disc is avascular (no blood supply) so it won’t heal. However, inflammation causes pain and over time inflammation generally reduces unless the provocation continues (heavy lifting, impact activities, etc..). The key to preventing flair-ups is to keep the core strong and alter your lifting and loading patterns of the spine.     Surgery is not commonly needed for your condition but you might find that epidural steroid injections can be quite effective.
