    In your eyes, what constitutes fusion ?

    I had TLIF at L5,S1 with BMP for a grade one spondy. Surgeon said I fused however still feeling wobbly on my feet and get some pain with too much exertion.

    I can walk much, much farther than before (approx 2500 – 5000 steps/day), but still painful to lift heavy items and and too much exercise.

    Radiologist says “incomplete bony consolidation” and cage is not “incorporated.” NO lucency in hardware. I have No leg pain or symptoms. Flex/Ext X rays show no lucency and no motion from previous study.

    Also, I had CT scan on 128 slice with 3mm cuts.

    Please let me know your thoughts. I am not where I want to be with my recovery.

    Normally, a CT scan for fusion status is on a 128 slice scanner with 1mm cuts only through the fusion site. 3mm cuts make it harder to determine fusion mass.

    Fusion is defined as cortical struts which connect one endplate to the other within the disc space. Fusion can also occur through the facets where the cortical struts should connect within and on the sides of the facets. The comment that “incomplete bony consolidation” and cage is not “incorporated.” mean there are no bony struts. No lucency means there is no haloing (looseness) around the pedicle screws.

    How far are you out from surgery? Was BMP used for the surgery?

    BMP was used. I am 13 months out. I do not see cortical struts when I look at it, however the fusion mass (woven bone) is definitely growing since last scan and has filled almost the entire disc space.

    I am somewhat concerned that the CT scan was too grainy and that is why cortical struts can not be seen. Either way my symptoms are not that much better from before the surgery; I am not satisfied with the result, althought I have not started PT yet.

    I think I am going to overnight my CT scan and other films to you for your review. Please let me know if you will be able to review the 128 slice 3mm cut or do you think I should have repeated on a high resolution machine???

    FYI, there is 320 slice CT in my area that can do 1mm cuts. I am 6’1 – 228 lbs – I am concerned with too much radiation. Please let me know your recommendation.

    The number of slices reduces your total time in the scanner so motion artifact is reduced, not radiation exposure. You only need the L5-S1 level imaged so you can reduce exposure by 80% with collimation. The images you have currently might be “OK” but are probably not fully diagnostic.

    I am told by most CT imaging centers that they can only to do full lower Lumbar study and not just L5,S1.

    I will send CT images to you and let you decide if I should get a new scan.

    1.) Please tell me what slice scanner you recommend and what cut ?

    2.) If there is no lucency in hardware after one year and no motion in Flex/Ext – is this a good indication ?
