    If there is white in the bony endplate, this could be IDR (see https://neckandback.com/conditions/isolated-disc-resorption-lumbar-spine-idr/). You can look at the STIR or FS (fat suppression) sequence and if the endplate lights up, it is definitely IDR.

    I recommend epidurals as they can be quite helpful and do not cause significant degenerative changes if used properly.

    Your description of IDR does sound like it could be what I have. Although my vertebrae don’t show quite such a large area as white. I do find running and even stepping of a kerb can be painful.
    I don’t think that I have STIR or FS images of my scans.

    On a recent report, it mentioned that I have osteosclerosis of the vertebrae in the thoracic spine. That is not where the white on the vertebrae is though. That is at L2, 3 and 4.

    White at the endplates would be either fluid from endplate fracture or fatty replacement (Modic type II changes) from chronic injury and fat replacement. If there was bone thickening, it would show up as a black thick area on the endplate.
