A good spinal rehab clinic should work on core strength, balance, ergonomics (how to position yourself to avoid overloading the spine) and understanding how to avoid postures that put the spine in precarious positions. This means directed exercises and specific activity avoidance.
The rehab place you went to sound like they specialize in expensive machines that are not very effective. The decompressor is essentially a traction machine which was dropped in the 1970s as an effective treatment. It was resurrected in the 2000s in my opinion for financial gain.
Shock wave therapy was developed for crushing painful kidney stones. Its use in spinal rehabilitation is questionable.
Find a center where you show up and sweat by working your muscles and then learn how to avoid injury.

i had review the spine surgeon again with new MRI contrast, he recommend surgery (redo discectomy) but he put me in new scenerio according to him he says that ur backpain will still be there after surgery while the leg pain will be releived,, Sir whats ur opinion regarding this?? second herniation at the same level will not releive the back pain?? although my back pain is much more than the leg pain,,
thanks u in advance
malikLower back pain generally originates from the disc and leg pain generally originates from nerve compression. If you have a recurrent disc herniation but have more back pain than leg pain, I would agree with your surgeon. A redo microdiscectomy should improve the leg pain but the back pain will probably not improve much.
However, if your back pain symptoms fit with degenerative disc disease (see website) and you have more back than leg pain, a workup to determine if you are a candidate for a fusion would be in order. A fusion (lets say a TLIF-see website) would improve both back and leg symptoms.
