Hi Dr. Corenman. Thank you for this forum. I have read several of your answers to many folks who are going through some trying times.

Nov 14 2018 I was weight lifting over head press and started losing strength quickly after that. This was followed by intense pain in my shoulder blade and my left trap became inflamed and guarded the neck. I was then rear ended in a minor car accident a few weeks later.

For the first few weeks I thought I was in need of adjustment, however nothing improved after chiropractor work. Nearing Christmas I went to an orthopedic who said my neck & shoulder xrays looked fine, resume light weight working out, go to physical therapy and take steroids.

January – After no relief I went to a neurologist and had an EMG and MRI. My C7 nerve root is compressed by a C6C7 Herniation 7mm & C5C6 3mm. I am not in pain nor do I have tingling or numbness. My main issues are weakness, loss of strength and muscle atrophy. I was told to give it 4-6 months to see if these issues improved. After minimal improvement over two months, I decided to see a Neurosurgeon.

In the beginning, I was unable to do a single push up, or dip. I could not extend an 8lb weight over my head. (Tricep over head extension) My left latissimus dorsi, pectoral and tricep have all gotten smaller and atrophied. I am gaining strength since the initial injury, but still very weak compared to pre injury strength. I can now extend an 8lb weight over head 50+ times, a 10lb weight 45times and a 15lb weight over head 10 times.

I am scheduled for surgery on the 29th of March for an ACDF 5,6,7.

Since the inflammation of the nerve has calmed down, is this why my strength levels are gaining? (Slowly)
Is the nerve still there, just compressed like a water hose?
Will it improve after removing the herniation? Will the muscle come back?

Was the damage to the nerve from the initial injury and is the nerve is in recovery now and will only continue to recover after the pressure is completely removed (disc removal)?

I am in no pain at this point and have really chilled on weight lifting to calm this nerve down. I do have a lot of exercises statuses recorded for comparison. I can feel the nerve sometimes similar to sciatic nerve pain. I also do not get sore in my muscles that are affected after working them out.

Thank you again for your time,
    Rayb

Cervical nerve have a better chance at recovery as compared to lumbar nerves. Herniated discs also generally recover better than lumbar nerves. You have gained some recovery at two months so you can expect some further recovery even without surgery. Nonetheless, surgery to decompress the nerve root will give a better chance to have full recovery than not.

I don’t think it would be a mistake to wait another 6-8 weeks and test your recovery as you have noted with dumbbell presses. Weight and repetition is the best measure of recovery and you have accurately measured your triceps improved muscle strength. If you continue to improve, you can continue to hold off on surgery.
