    Do you recommend that your patients wear a back brace after spinal fusion at L5,S1 or slowly start building muscle using a walker?

    What is your protocol ?

I have every lumbar surgery patient wear what I call a “corset”, a firm fabric stretchy brace that increases abdominal pressure. This allows mores stability of the trunk muscles in a period where the muscles are initially dysfunction, After about 6 weeks, the muscles can re-engage and there is no need for the brace. It is a very rare case that someone needs a walker.

    Can you please tell me the name of the corset that you use ?

    I was wearing an Aspen Quick draw (I removed the plastic in front) for years before the surgery. It is mostly nylon with some rigid plastic in back. I did not always connect it up front, rather just stick it in my pants for stability when I needed it. Seemed like it helped support my hips more than anything.

    In any event, my surgeon doesn’t want me using any brace at this point after surgery, which is causing me to become more sore than usual and increase pain meds and use walker. They surgeon claims that it is important for muscles around spine to strengthen while fusion is occurring.

    I just don’t want to mess anything up with the hardware or fusion if I decide to put the brace back on for comfort.

    I am 6’1, 225 lbs so I think I need all the help I can get to ambulate while fusion is taking place.

    Please let me know your thoughts.

    I don’t want you to go against your surgeon’s advice for brace wear. All I can tell you is that I use it for six weeks after fusion. I use a Miami brace but an Aspen brace would be acceptable to me.

    Are straight leg raises acceptable
    after surgery ? And how far out from surgery can I start stationary bicycling ? Will bicycling help the fusion process ?

    I seem to be getting lot of tightness and soreness on non TLIF side after surgery which can travel down the leg with walking? I did not have these symptoms before surgery. It’s only been 8 days from surgery but I would like to cycle instead.

    I will be following up with surgeon in the morning however your guidance is always valued and appreciated.

    I can only discuss my protocols. I let patients get on an upright stationary bike or elliptical immediately after surgery. Biking will not help or hurt the fusion process but will help with recovery (conditioning and cardiovascular improvement). Straight leg raises (not hamstring stretches but lying down and lifting your leg up) I frown upon for 6 weeks to three months as this places the fusion into stress.

    You might have some root irritation after surgery and if I find that to be true, I occasionally will put a patient on an oral steroid for 5 days if that is noted.
