Hi Dr Corenman,
I have a large AVM in my back, which I have lost a lot of muscle from.
I have been having a different pain in my left lower back for a few years, but in the last 6 months it has been very painful and shooting down my leg a lot, as well as breakaway weakness. I have been having physio for months and have had so luck. I’ve had a recent MRI which showed a mid-line disc bulge, which my specialist has said will be causing my symptoms. We are going to try 6 weeks of physical therapy again, but I’m worried it won’t do anything, as I have already tried for months. I’m very happy that surgery isn’t an option but I feel a bit lost as to how to make this better. I can no longer do any exercise and am struggling with my studies because sitting hurts so much. I have tried to research but cannot find anything online. Is the AVM (arteriovenous malformation) in the back muscles or in the spinal canal? I assume the back muscles.
Midline disc bulges without discal collapse or herniation can cause back pain but should not cause leg pain. See if either of these two conditions might fit the bill- https://neckandback.com/conditions/lumbar-foraminal-stenosis-collapse/ or https://neckandback.com/conditions/herniated-disc-lumbar-spine/.
You might have a condition that is present but undiagnosed. Do you have the radiological report?
