    I have similar question as Haresh.

    I also have a sacralization at L5,S1 which I believe would technically make my pars fracture segment at L4,L5 although most Dr’s refer to it as L5,S1.

    If I were to successfully fuse at the segment above sacralization and all pain generators were removed, i.e facets, disc, pars fracture could I still have pain down the road at segment about or below or anywhere else ?

    If you have a sacralization of L5-S1 and pars fractures at L4-5 (isthmic spondylolisthesis), it would be highly unlikely that you would have pain in the lower segment. It can happen (see https://neckandback.com/conditions/bertolottis-syndrome-colorado-spine-surgeon/) but is very rare.

    Now you could break down at the level above (L3-4) and develop pain. The chances of that are approximately 2.5% per year (or 25% chance in 10 years).

    Ok, so once this fusion is done I should not have any other pain generators except for L3-L4 ?

    I will assume that L3-4 is not a current pain generator. If it is not a current pain generator, the chance of L3-4 becoming a problem is 2.5% per year. Now can any level, the sacroiliac joints, the hip joints or even the thoracic spine eventually cause pain? The obvious answer is yes possibly eventually.

    I forgot about the sacroiliac joints. A Fusion at L5,S1 or in my case L4,L5 (with sacralization) can eventually pull on the sacroiliac joints causing pain, correct ?

    How is sacroiliac pain diagnosed ? What is done about it after fusion? Thank you very much !
