    try to be brief. 6 years. started as aching pain behind thigh. progressed. sometimes with low bk pain a lot of time no back pain. have had 3 MRI’s overtime. Myelogram. spinal steroid shots. Just had MRI 3 wks ago, dr. said disc bulge. no help. Tried therapy hurting more. Went to ER got steroid pk has not helped but did in past. I get up usually pain free from sleep, the moment I sit, right buttock touches car seat, chair, pain and horrible. goes into my rt flank. down my leg as far as the foot. It feels like im being cut with a razo and alcohol poured over it or a million be stings! My life is seriously curtailed I almost at point of cannot work. They look at low back only nothing more. The nerve is pinched I am sure but can anything else mimmick this horrible pain? Please help.

    Your symptoms fit perfectly with a disc herniation in the lumbar spine. The nerve feels better when lying down or when standing and hurts with sitting. The pain is in your thigh and radiates down to your foot.

    This pain pattern could be from either L5 or the S1 nerve compression.

    If you have had the pain for six years, it seems that no further “tincture of time” will be effective to reduce pain. If you have an obvious nerve compression from a spur or herniation, this might be the time for a microdisectomy.

    With six years of pain, I worry about chronic radiculopathy but if you still get relief from standing or lying, the chances are still good that a surgery will help you out (assuming there is compression of the nerve now)

    Let me explain further. I use to jump from my car for relief. Standing always helped. I have progressed beyond that. Standing will stir it up as well. If I am moving I am usually ok for a while, Sometimes long periods of activity make me hurt some. It use to come and go at first not anymore, its constant unless I lie down. I want to work out so badly and enjoy life I just don’t know what to do. Why does it automatically stir into my side flank? I feel great other than this I just want to enjoy life again. Dr. said slight spinal degeneration and bulge at L5. He says no surgery ?? Should I seek further help?

    Oh yes and sometimes the pain is in the saddle area as well

    I think that you need a second opinion. I am worried that the pain is now more constant. You still need a complete history, physical examination and evaluation of your images. You also might need a selective nerve root block. I cannot tell you that you are a surgical candidate but you certainly need the opportunity to know if surgery can help you.

    Thank you Dr. for for your help. You are amazing that you take time to help others. I just need help surely someone can help, it just seems they don’t care. I wish I knew some things I could do to manage pain better for now. Dr. said therapy excersises can help push disc back I don’t buy that.
