in reply to: Disc Bulge after Thoracic Microdiscectomy #33578
Dr. Corenman,
Thank you for your thoughts. I had a meeting with my PA, and she believes that I simply aggravated the nerves, which has led to my setback. I have been in more pain than before PT for about two weeks, but it is starting to settle down. My PA believes that I have not reherniated because my symptoms slowly build up, and I have slowly gotten better. However, my PA suggested an injection to help the inflammation settle down. I have two questions, though:
1. I know that a thoracic microdiscectomy is a little risky simply because it is unlikely to herniate a disc in the thoracic zone. Should I be worried that an injection could severely injure me since it would be in the same area? Should I seek out a specialist for this injection?
2. I have had allergic reactions to prednisone in the past; any dose over 10 mg causes heart palpitations. Is there any type of injection that would be safe and would not cause complications? I don’t know if I have any other allergies to steroids at this point.
Thanks for all the help. You probably hear it all the time, but you’re the man and greatly appreciated.

As a millennial, I definitely struggle with patience! It’s not one of my strong suits. I do have a question about the concept of patience, though. I know that nerves can take 6-12 months to heal, which totally makes sense. At my 3 month mark, I am still experiencing flare ups that last anywhere from a few days to a week. In your experience (and I know everyone is different), when do most patients start to drift out of the up and down pattern into a more consistent period of healing? I know that I feel better since before surgery, but it is still exhausting going through the up and downs, and I feel as if I won’t get better. Do your patients start to see an upwards healing process in months 3-6?
Thank you for answering all my questions!

Thank you for your quick reply! I was just nervous because I just started doing physical therapy, and my pain immediately came back after it almost completely stopped, which made me nervous. I am sure everyone feels that way, though, after physical therapy initially.

Thanks for the information about the disc bulge. My surgeon did not explain it as clearly. However, I have another question. Pre-surgery, I had a lot of pain in my ribs, and I had these awful rib spasms. I haven’t had one for five weeks, but yesterday I had PT, and I nearly collapsed this morning from incredibly painful spasms. Do you have any recommendations on what I should do next?

I had my follow-up with my surgeon, and it went really well. He informed me that most likely, my disc bulge will go away over time. Therefore, I have two quick questions: 1. Why do I have a disc bulge after my microdiscectomy and why is that common? 2. What is the likelihood that a bulge will go away, recede, or stop causing problems?
