The artifact of C45 is more to the reason C4 is shaved off 1 cm in the surgery ( not sure why). is it possible to do an osteotomy and put a 1 cm high graft in the C45 (the current C45 is 2mm. Its creating an abnormal structure. I have a couple of questions
1) Now that the surgery is done for a few months, Does it mean height increase would not fix the dispositions of the lamina?
2) There is probably no device like zero profile device or ADR that can fix a 1 cm height issue.
3) For autograft, taking a 1 cm from the iliac crest, would it be more painful because it is so big?
4) I saw a write up that says allograft has 92-95% fusion rate as opposed to 95-98% in autograft. Do you think it would be different for this case?
5) Will waiting for a month or two make it even worse? Should it be fixed ASAP because the structures will become more permanent?in reply to: redo a imperfect surgery #26308Dear Doctor Corenman
I have an update. One of my levels C45 had loose screws and now it seems that the C4 and C45 collapsed and it created a kyphosis. Is it possible to fix this kyphosis aneterior way? As it is less than six months old,is it possible to do osteotomey with allograft (it looks like it needs a large bone). I have not been able to do an SNRB. I have foraminal stenosis at another level which makes it even more complicated.
These are the difficulties I have hard
1) Anterior osteotomy & fusion may not work because screws will have little purchase. So a posterior fusion would be needed.
2) Anterior surgery has to be done with autograft. I am a bit worried about permanent pain/issues on autograft.Both posterior approach and autograft are known have higher degree of permanent pain and issues than ACDF with allograft.
I guess osteotomy with allograft is not option? Spacers (zero-profile or not) or ADRs are not possible as well I would guess.

Now that I am fused. Should I get an anterior surgery to open up both foramens? Is it still possible to do allograft (almost 4 months)? Is it better do it early? Or late (like in six months)?in reply to: redo a imperfect surgery #25714Doctor Corenman,
It has been almost 3 months. IF the redo happens within 4 to 5 months after the surgery do you think Allograft would still work?
