Thank you for your kind reply. Yes after each nerve block I have hsd significant relief for a few hours .. After my second nerve block into L5 S1 I had (after a two week delay) four months when I was virtually pain free ! Doesn’t that indicate that the source is the nerve root ? What kind of tests should they be doing to confirm/deny peripheral neuropathy! I have no other symptoms than severe foot pain – no numbness, temperature changes, hypersensitivity to touch … Do you think it could still be a peripheral neuropathy (in one foot only ) ?
Thanks for your kind advice !

So this is the post surgery MRI form two weeks ago – my foot and coccyx pain are currently excruciating . Do you think a new MD would help ?
PS I don’t have any degenerative disc disease do I ? They would have mentioned it right ?

Clinical Indication Right L5-S1 decompression 2 weeks ago. Right foot pain worse.

Report The moderate left-convex scoliosis is unchanged. There is evidence of a recent surgical approach with fenestration of the right L5 lamina passing through the medial aspect of the adjacent L5-S1 facet joint. A new nodule of tissue is now seen just lateral to the right S1 nerve root at the anterosuperior margin of the facet joint resection. This may well be causing some low-grade radicular impingement and the nerve root itself appears slightly swollen compared to the preoperative imaging. The L5-S1 disc has not altered substantially. A small collection lies within the subcutaneous tissues superficial to the L4 and L5 spinous processes. The small far lateral disc protrusion at L3-L4 is slightly smaller than on the preoperative study.

No new pathology is demonstrated elsewhere. Conclusion There is a small nodule of tissue just lateral to the right S1 nerve root, interposed between the anterior margin of the partially resected facet joint and the adjacent L5-S1 disc. This could represent debris from either the facet joint or the disc and is a potential source of mild S1 radicular impingement.

One very very last question – assuming this second MD works , how long does it take for an inflamed, swollen nerve root to calm down and resume normal service ? What can be done to help it along ? Thanks .

    Thank you so much !
One (very) last question ! : The surgeon says that my herniation was small and that the post-surgical ‘debris’ now near the S1 nerve is small. Nevertheless I have ABSOLUTELY excruciating pain . He says my nerves are ‘ overly sensitive ‘ and that they reacted to being manipulated during the discectomy . If I have a repeat discectomy , what can be done to ensure that similar manipulation doesn’t result in even more nerve root pain and swelling . I am very afraid that a second surgery is just going to make the problem worse as I seem to have such sensitive nerve roots. Any thoughts on how to prevent such a reaction ? Thank you for your kind help !

Hi again , one more thing ..the coccyx pain appeared out of the blue a month after the discectomy – as the S1 nerve is irritated, can this not cause coccyx pain ? I am now worried that I have something else going on ! But the fact that the coccyx pain went after the L5S1 nerve root block is reassuring right ? (Ie indicating it is caused by the nerve root ) . If I had regular ‘low back pain’ this would not be relieved by the nerve root block would it ? Sorry to bother you about all this but could you give me one last clarification on all this please. Many thanks indeed.
