Hi Dr corenman. I am 20 months post-op acdf c456. Post-op MRI showed a very bright spot in my spinal cord. It said myelomalacia and myelopathy. Is there something wrong if I am having worsening numbness and pain “neuropathy”. My fingers and hands and legs and feet from knees down are getting progressively worse for the past four months. Should I be rechecked with MRI

You most likely had cervical stenosis (narrowing of the central canal) that caused cord compression and injury. The signal change you note in the cord (“very bright spot in my spinal cord”) is this injury. When the cord is injured, the tracts “drop out” causing cord narrowing (myelomalacia).

If you have progressive symptoms, a new MRI is warranted.
