    hello doctor hope you are allright, my name is alonso i been having low back pain and weakness starting 3 yrs ago but getting progresively worst,so much that i cant stand,sit,walk for more than 15 mins before i have to lay down or put my upper body over something to relieve pain and loss of power,also im strangely bent at left side.for the past 4 months i been in a crisis of severe pain ,wich been receiving epidural shots(2) so far they have removed the excrutianting pain in the right side and thighs and right side of back but i still have left lower back pain and pain in left groin tingling in genital area and outside of left leg pain ,tremors and burnig acid sensation.had an mri done and among other stuff it said that l4l5,l5s1 had severe foraminal narrowing bilatreal more so on left side impinging on l4 l5 nerves bilateral on both but more so on left side impinging on left l5 nerve periferaly my question is could these be causing the symptoms and what will be the indicated surgery to fix this problem thank you in advance eagerly awaiting answer good day!

    The inability to stand upright with symptoms down the legs (buttocks, pelvis, legs) typically is stenosis or narrowing of the central canal (central stenosis), lateral recess stenosis or foraminal stenosis. The latter two cause compression of the individual nerves. See the website for description of these symptoms.

    If you have foraminal stenosis due to discal collapse (severe narrowing of the disc space called isolated disc resorption (IDR-see website), typically you need a TLIF fusion. If there is still some disc height preserved, sometimes a foraminotomy (no fusion) can be performed.

    If you have central or lateral recess stenosis, a laminotomy can be performed (opening of the canal without fusion).
