Dear Dr. Corenman,
My name is Dan I am a very active 30 year old person that went to the gym 4 times a week before surgery, I had a microdiscectomy on L5S1 10 weeks ago on my right side due a severe herniated disc causing sciatic pain down the right leg after standing for long periods of time.
My surgeon basically told me at 5 weeks that I could resume my regular activities with moderation and that I should continue physical therapy. I have been on PT since week 4 focusing on my core and my back without any pain during the exercises.
Right after surgery I had paresthesias on both legs, lower back pain which I did not complain before surgery, and right and left glute pain. All of those symptoms have improved progressively but I still have some symptoms.
Currently I have not had the same sciatic pain as before surgery, I have minor twitches and spams on my right soleous muscle and on my right external gastrocnemius (these sensations are my main concern, because my right external gastrocnemius is atrophied when compared to the left gastrocnemius). I also have some discomfort on my right glute when I sit for 45 min or more and when I sleep on my left side with a pillow between my legs. My lower back around the incision area feels stiff in the mornings, and after standing for 1 hour or more without sitting causes some pain. Also when I walk I have noticed that I feel my lower back muscles spasm sometimes and some incision pain that has decreased progressively but there is still some lower back pain as mentioned before.
I do not have any weakness with dorsiflexion, plantar flexion, and with walking. My right Achilles tendon reflex is negative and has not returned yet. Straight leg raise test is negative on both legs. The slump test causes minor muscle contraction around the incision area but no leg pain.
I have not done any BLT at the same time. Mostly just bending and only during PT, for everything else I squat or kneel down which causes my lower back muscles to contract but it is not painful, just a weird sensation.
My concern is if everything that I am experiencing at 10 weeks is considered normal or if I should be worried about a possible Reherniation or Failed Back Syndrome. I am not taking any painkillers because I have not felt pain so severe that warrants taking painkillers.
I want to be able to have a normal active life again without worrying about a failed surgery.

DanYour symptoms after microdiscectomy is not out of the margins. There are times it can take as much as 3 months to have the nerve return to normal. There is a possibility that you have developed a seroma, a fluid collection that occurs after some surgeries that continues to irritate the root. This fluid collection should resorb over time. If the symptoms are currently disagreeable, ask your surgeon for a 5 day course of oral steroid.
