Hi Dr.Corenman,
Past few months, I had problem with numbness or more of numb sensation in right foot and weakness in legs. So i decided to do some research on google on what could cause it and I came across cauda equina syndrome<. Ever since then, I was paranoid about getting cauda-equina-syndrome. Currently I am feeling less sensation in my private area and anus then I used to. It is hard to get an erection and I dont konw if it is caused by worrying about cauda equina syndrome constantly. Also, when i clench my anus it seems weaker?, i am not sure if it is due the fact that I am constantly looking for new symptoms. I went to my college doctor and she said it might be due to piriformis muscle tightening and recommend me to PT. When I went to physical therapist, she told me that there is some instability in my deep muscles in my lumbar region. I am currently doing the exercises and stretches she told me to do but I still have this feeling that I might have cauda equina syndrome. are the symptoms i have most likely to be cauda equina syndrome and get an mri or should i continue my pt? I am really worried. FYI-I am 23 years old college student and my daily activity consits of lot of sitting and 1 hour of workout at the gym. I have no prior back injury or surgery.

It is highly unlikely that you have cauda equina syndrome. This typically comes on with a severe and immediate onset of pain, numbness in the saddle area (where your skin would touch a saddle), weakness and loss of bowel and bladder control. Patients are typically in extremis (severe pain and panic). The examination demonstrates significant loss of bowel tone and patients cannot easily control the bladder. Sensory loss (anesthesia or full numbness) accompanies these findings.
It is unlikely that you have this urgent condition but there are other neurological conditions that can mimic cauda equina syndrome. If you really are aggravated by these symptoms, I encourage you to seek out a neurologist who can work up your condition and hopefully find a treatment.
