Hi Dr. Corenman,
I am a 47 year old female recently diagnosed with an annular tear of L5 S1 Disc.
Symptoms started out as severe leg weakness, both legs, back on June 24th. I had tweaked my back several times the month prior to this, but did not realize I had actually injured my back to any degree of severity.
The L5 S1 tear wasn’t found until my MRI on August 29th. Prior to this, I had seen numerous Dr’s trying to determine the cause of the leg weakness and was, and still am, having to brace my knees, as my legs feel as though they might buckle after just a few minutes of trying to stand/walk. I was also experiencing pain in my ankles and was having to wrap them also, as they felt as though they would collapse inward.
I also had an MRI of my thoracic spine and will include all findings below.
My question is; can any of these findings on MRI be the cause of my leg weakness and difficulty walking? I have seen two orthopedic Dr’s thus far, and neither seems to think it’s related, and I don’t really understand why. I am also expeiencing pain in my hips and pelvis/groin and am now having trouble sitting for any length of time due to pain in tailbone and buttocks (within the last 4 weeks), almost as though my SI joint is also affected and so am awaiting approval for an MRI of my pelvis next.
Here are the findings of my THORACIC MRI, with and without contrast:
The vertebral bodies are normal in height, configuration and signal intensity.
There is normal bony alignment.
There are no acute fractures.
There is presence of an approximately 8mm right T2-T3 perineural cyst.T4-T5: There is a left paracentral disc protrusion resulting in slight deformity of the vental aspect of the spinal cord.
T5-T6: is a left sided disc protrusion but no significant stenosis or spinal cord compromise.
T8-T9: There is central disc protrusion but no significant stenosis.
The Spinal cord is normal in size and signal intensity.
There are no paraspinal masses or collection.IMPRESSION: Disc protrusions as described above.
Here are the findings of my LUMBAR MRI without contrast:
For the purposes of this dictation five lumbar vertebral godies are assumed. Conus normal in signal inensity terminating at the L2 level. Nerve roots of the cauda equina unremarkable. Vetebral body heights maintained. Nonspecific straightening of the normal lumbar lordosis. L3 vertebral body hemangioma.
L1-L2: No disc bulge/herniation or central or formiinal stenosis.
L2-l3: Mild disc dessication and small Schmorl’s nodes.No disc bulge/herniation or central or foraminal stenosis.
L3-L4: Mild facet degenerative changes. No disc bulge/herniation or cntral or foraminal stenosis.
L4-L5: No disc bulge/herniation or cntral or foraminal stenosis.
L5-S1: Disc dessication. Small central disc protrusion with annular tear without central or foraminal stenosis.
IMPRESSION: Mild degenerative changes of the lumbar spine with annular tear of L5-S1. No significant central or foraminal stenosis.
Would appreciate any thoughts you might have regarding my MRI findhings and current state. Wondering if possibly something is being overlooked or missed. Thank you!
