3 weeks out with continued similar pain after a microdiscectomy could be from a missed fragment (incomplete decompression), a new fragment (recurrent herniation), from a seroma (a collection of blood that occasionally occurs from surgery or continued nerve root irritation. The first thin to try is an oral steroid. If that is ineffective (and your physical examination has not changed), the next step is a new MRI. Missed or recurrent fragments mean a redo surgery. A seroma needs an aspiration and steroid injection.
