    hi, dr i have been having deep stabbing pain in my mid back post c6/c7 l3/l4 fusion 3years ago the pain will come on and will unvulantary jerk my body on 9/4/13 mri findings were posterior extruded left paracentral disc herniation at t7/ t8 effacing ventral CSFspace impinging the left anterior margin of the cord.Posterior CSF space and neural foraminal are patent. No additinal focal thoracic disc herniation seen. No other levels of central foraminal stenosis. how important is this.

    The importance of a thoracic disc herniation depends upon the symptoms that the hernaition generates and any problems with dysfunction of the spinal cord.

    Pain and numbness from a thoracic disc herniation can generally be managed with physical therapy and epidural injections. Time is also a great treatment tool as generally (but not always) the symptoms will diminish with time.

    If there is spinal cord dysfunction (myelopathy-see website), the story is different. The presence of myelopathy is confirmed by both a thorough history and a precise physical examination along with the imaging findings. Generally, the presence of myelopathy dictates a decompression surgery with a fusion of the decompressed level.
