    Dr. Corenman,

    I will! I do have a question as a follow-up. My flare up pains have mostly gone away for small exception–my upper rib cage on the right side hurts terribly. It started occurring right when I got hurt at PT. The pain is nearly constant (it’s a throbbing pain), and then it switches to the middle of my left rib cage. I had surgery on my T11/T12, so this pain is above the surgery site. Is it possible for a flare-up (or reherniation if that is the case) to cause pain or nerve issues higher than the injury site?

    Also, I have a ton of burning sensations since switching to Lyrica. I *think* I am starting to improve, but it’s super strange and slow. I don’t know if flare-ups are supposed to last more than 4 weeks, but I am starting to get concerned that I have reherniated instead of having an aggressive nerve agitation.

    I’m not quite sure why Lyrica caused a “burning” sensation. Give it some time to improve. If you are going to get an epidural injection, you should have a new MRI first.

    They want to do the ESI first. Should I request a MRI first?

    In my practice, I want to know what the injection purpose is for. Do you have a seroma which requires an aspiration with an injection or has the fragment migrated requiring a different approach (rare). It is not wrong to do an injection without imaging but less efficient and possibly less effective.
